Effects Of Furosemide Administration On The Effective Half Life Of Radioiodine And Radioiodine Uptake In Graves' Disease

Ankara Oncology Research and Training Hospital, bDepartment of Nuclear Medicine, cDepartment of Endocrinology and Metabolism, Gazi University Medical School, Ankara Objective: The aim of this study is to evaluate the effects of furosemide – a loop diuretic- on radioiodine (I-131) uptake in the thyroid and effective half life of I-131 (Tef) in patients with Graves’ disease. Material and Methods: In this study, 33 patients with Graves' disease (6 male, 27 female, age: 44.7 12 yrs) were examined. I-131 uptake test was performed and Tef of iodine was calculated. After two weeks, 40 mg furosemide p.o was administered for 7 days, I-131 uptake and Tef were re-calculated. The results were correlated with baseline levels. Results: After furosemide administration, significant increase was observed in I-131 uptake in general (p: 0.002). An I-131 uptake increase post- furosemide was observed statistically significantly in patients with baseline 24 h I-131 uptake < 30 % compared to those with an 24 hr I-131 uptake > 30%. Tef was significantly lower in patients receiving antithyroid medications than patients without treatment. No significant changes between baseline Tef and post-furosemide was observed. Conclusion: This study showed that furosemide provided significant increase in I-131 uptake without any change in I-131 Tef. The increase of I-131 uptake after furosemide use was more pronounced in patients with low baseline 24 hr I-131 uptake compared to patients with an higher one. In consequence, furosemide administration might be useful to optimize I-131 treatment in Graves’ patients with low iodine uptake.Keywords: Graves Disease; diuretics; iodine radioisotopes; half-lifeTurk J Nucl Med 2009;18(2):41-6
